Congratulations on completing your course/school term. To receive your bursary payment, complete the Bursary Payment Request form before May 15th for Winter/Spring awards and before Dec. 15th for Summer/Fall awards.  Be sure to include/attach all required documents and double check the contact details for accuracy. Missing or incorrect information may delay your payment or not allow us to process your payment. Please note that we may use some of your response(s) on our web site or other media outlets to promote Balding for Dollars.

If the form is not visible below, we are no longer accepting payment requests for the past term and the form will be re-opened for the next award period.
